also write "apply for adoptive parent", and send it back to us
together with a ￥90 postage stamp enclosed to:

When you fill out the application form please read the agreement enclosed
very carefully again. Please contact us if you have anything unclear.
Request for the documents by e-mail is also accepted, but it may take some
time because we do not have an office staff member who works exclusively
on office works.
The e-mail request for documents mentioned above is purely for confirming
the adoption applicants' addresses and names for us to send out the application
forms.
Please note that adoption application is not accepted by e-mail.
If you want to be an adoptive parent and request for the documents by e-mail,
please clearly mention your request for an adoption application form together
with your address, name and telephone number.
To save costs, we would be happy if you could kindly apply by post with
a postage stamp enclosed.
